PROCEDURE TO GET A CGHS CARD
Eligible serving employees/pensioners have to apply online or in the prescribed form. The form should be completely filled up with individual photographs pasted as specified in the form. Following documents need to be attached:
In case of Serving employee:
- Proof of residence.
- Proof of age of son.
In case of differently abled dependent son above 25 years, disability certificate from competent authority as specified.
In case of Pensioners, in addition to 2 & 3 above:
- Surrender certificate of CGHS Card (only if CGHS Card was issued during Service Period).
Bharat Kosh transaction receipt. All the receipts of the Government of India shall be done through bharatkosh.gov.in under Non-Tax Receipt Portal only. Henceforth, all the receipts towards CGHS subscriptions from pensioners for issue of pensioner card, renewal of cards and issue of duplicate cards shall be received through Bharat Kosh in NTRP only. Fine of Rs 30/- is applicable for delay in submission beyond three months post retirement.
Certificate regarding Non Drawal of Fixed Medical Allowance (FMA) should be mentioned in the PPO/ LPC/ or recent certificates should be obtained from the bank.
Requisition for Pensioner’s CGHS card(s) shall be accompanied by three copies of recent 3 x 5 cm size individual photographs of all family members of the retiring employee, one set of which shall be pasted on the application form and shall be attested by a Gazetted Officer in charge of administration (other than CGHS). Another set of photographs shall be signed on the back by the concerned beneficiary and enclosed with the application form for onward submission to the Office of Additional Director, CGHS Bangalore.
The amount of Demand Draft will be for contribution due for one year if the card is to be made for yearly renewal basis and for 10 years if whole life card is required.
DEPENDENCY CRITERIA
For availing the medical facilities under the scheme, parents (or parents-in -law in case of female employee), unmarried son till 25 years of age, dependent unmarried/widowed/divorced/separated daughters and sisters, minor brothers are deemed dependent on the Government employee if they are normally residing- with him and their income from all sources including pension and pension equivalent of DCRG benefit is less than Rs. 9000 + DA per month. This criteria does not apply to spouse and disabled son irrespective of age (please see definition of disability in instructions for filling CGHS card).
CHANNEL FOR SUBMISSION
In case of serving employees the application is to be submitted through the department, duly sponsored by the sponsoring authority. In case of Pensioners, the application with relevant documents and enclosures are to be submitted to the Office of The Additional Director, CGHS Bangalore.
PROVISION FOR MAKING PENSIONER CGHS CARD WHILE IN SERVICE
A serving employee can apply for a pensioner CGHS Card along with his pension papers.
Application with enclosures and bank draft is to be forwarded to the CGHS through the office of the employee. The pensioner Card will be issued on the day of retirement (provided it is applied for at least six weeks before retirement date) and will get activated from next day.
ONLINE APPLICATION FOR CGHS CARD
The card can be applied online by visiting the link. However at present provision for uploading the supporting documents is not available. After online submission of the application, the applicant is required to take a print out of the application and submit it with supporting documents (duly endorsed by the Head of the Department/Office in case of serving employees) to the office of the Additional Director, CGHS Bangalore.
VALIDITY OF THE CGHS CARDS
The service card will be valid till the date of retirement if otherwise the employee is eligible.
In case of yearly contribution pensioner cards, in order to continue validity, contribution is to be made prior to completion of the continuing year.
CGHS card is valid in all CGHS cities for treatment /investigation / Hospitalization. There is no need for transit permit to get treatment in another CGHS city except for receiving high value medicines classified as “lifesaving”/restricted supply medicines for which temporary attachment to a wellness center is required.
CGHS CARD FOR PENSIONERS RESIDING OUTSIDE CGHS CITIES
Pensioners residing outside CGHS covered area can opt for a regular CGHS Card or an IPD (Indoor treatment) CGHS Card with fixed medical Allowance (in lieu of OPD treatment) from the nearby CGHS city. IPD card holders will not be eligible for OPD treatment & issue of medicines from CGHS Wellness Centres.

CGHS CARD COLOUR SCHEME
CGHS Plastic cards have the following colour at the top:
CGHS Card
- Serving Government employees – Blue
- Pensioners, ex-MPs, Freedom Fighters, etc. – Green
- Member of Parliament – Red
- Beneficiaries of Autonomous Bodies / Journalist – Yellow
CGHS CARD RENEWAL
The cards are initially issued with a validity period of 5 years. The CGHS Plastic Cards completing their validity period are due for renewal and accordingly fresh cards with renewed validity period are issued by the Office of Additional Director, CGHS Bengaluru.
SERVING BENEFICIARY
Application for renewal of CGHS Plastic Cards in the prescribed proforma (Form AA) along with requisite documents (current photographs, copy of pay slip and address proof of residence, if changed), should be submitted through their Administrative Office to the Office of Additional Director, CGHS Bengaluru following the same procedure as prescribed for issue of fresh CGHS card.
Fresh CGHS Plastic Cards with same beneficiary ID nos. shall be issued with a validity period of 5 years.
Applications for renewal of old plastic cards can be made 3 months in advance prior to its expiry.
PENSIONER BENEFICIARY
Application for renewal of Pensioners’ CGHS Plastic Cards in the prescribed proforma (Form BB) along with requisite documents (current photographs, PPO or LPC, address proof, if changed) should be submitted to the parent CGHS Wellness Centre where his /her card is registered. He / she can also submit the application to the Office of Additional Director, CGHS Bengaluru for renewal of CGHS cards.
Fresh CGHS Plastic Cards with same beneficiary ID nos. shall be issued with validity for lifetime or up to the date for which the contribution has been made by the beneficiary.
Applications for renewal of old plastic cards can be made 3 months in advance prior to its expiry.
LOSS OF CGHS CARD
CGHS CardIn case the identity card is lost, the beneficiary should lodge a complaint with the police station and also report the matter to the CMO In-charge of the dispensary, the sponsoring Department/ Ministry, and to the Additional Director, CGHS in the city.
A duplicate identity card shall be issued if the original card is lost, by the authority who had issued the original card, on receipt of an application in this respect along with following documents:
Copy of FIR from police station.
Copy of complaint lodged with the police about the loss of original card.
Charges for issue of a duplicate through bharatkosh.gov.in under Non-Tax Receipt Portal favouring PAO, CGHS of the respective region, for a sum of Rs 50/- (in case of plastic card).
Two recent passport size photographs.
TRANSFER OF CGHS CARD
CGHS OfficeIn case of change in residential address and shifting from one Wellness Centre to another, CMO In-charge of the original Wellness Centre shall make modifications in data base and transfer the cards to new Wellness Centre. CMO In-charge of new Wellness Centre shall accept transfer of card(s) and data shall be transferred to new Wellness Centre. Click here to download the form.
In respect of serving employees the request is to be forwarded by his/ her office/ department.
It has now been decided, in supersession of the earlier orders in this regard, to allow online transfer of same CGHS cards of serving employees of central Government on transfer from one CGHS city to another CCHS city as per the details given under:
Serving employees on transfer from one CGHS city to another CGHS city shall submit an application to the Additional Director of the CGHS City (forwarded by the Ministry/ Department from where he /she is being transferred) along with copy of the transfer/ relieving order for transfer of CGHS Cards to another CGHS city with an undertaking that he shall be residing in the new city in a CGHS covered area.
Addl. Director, CGHS of city shall transfer the card online and shall issue an acknowledgement slip to the serving employee. CGHS Plastic cards shall be retained by the serving employee.
Serving employee in the new CGHS city submits an application duly forwarded by his/her Ministry/Department/ Office, certifying that CGHS contribution is being deducted, to the Additional Director of new CGHS city for acceptance of the transit CGHS cards to the data base of new city and allocation of a CGHS Wellness Centre in new CGHS city along with proof of residence for residing in a CGHS covered area. Additional Director, CGHS in the new city shall accept the cards in new city after verification of the residential address. If the area is not covered under CCHS, the card shall remain inoperable until it is accepted by the Addl. Director, CGHS.
It is also brought to the attention of all Ministries and Departments that they are responsible for surrender of CGHS cards issued to Serving employees when they are transferred to a non-CGHS covered city.
ADDITION/DELETION OF NAMES IN CGHS CARD
On the death of the main card holder, the card becomes invalid and fresh card has to be applied for by the spouse after he/she starts drawing the family pension. Old CGHS card and a Death Certificate need to be attached with the application.
A serving employee on marriage or on the birth of his/ her child may get the names of spouse /child added to the card after submitting the form for addition duly endorsed by his department.
After the death of spouse and death/marriage/employment of a son/daughter/dependent it is the responsibility of main card holder to inform CGHS for necessary deletion of the card.

SURRENDER OF CGHS CARD ON SUPERANNUATION / RETIREMENT OR TRANSFER
In case of superannuation / retirement, the plastic card becomes invalid on superannuation and he / she have to surrender cards of self and his family members to Department. Department shall issue a surrender certificate to employee for obtaining a pensioner CGHS card in case of superannuation/ retirement.
In case of transfer to another city, serving employees shall get the validity of card edited in data base so that he / she would be ineligible to obtain benefits from that wellness centre and obtain a certificate from CMO In-charge accordingly and surrender the card to his / her Department. The Department shall issue a surrender certificate to employee for getting a new Card at new city, provided that city is covered by CGHS.
Surrender of CGHS Card by Pensioner going out of station
If a pensioner having lifetime CGHS card issued at one place, shifts to another place then the card is to be surrendered in the office of AD of the first place & after obtaining surrender certificate and NOC, a fresh card shall be obtained from JD of the other place.
Pensioners shifting to a city not covered under CGHS or abroad, are required to surrender their identity cards to the concerned wellness centre against an acknowledgement receipt. However, contributions already paid shall not be refunded. On their return, they may get the card issued again on the basis of the receipt with them, without paying the contribution for the intervening period.
However, if the card is not surrendered at the time of shifting they shall be required to pay actual contribution for the intervening period or a readmission fee of Rs 30, whichever is less.
SELF-PRINTING OF CGHS CARD
Beneficiaries can now print their own cards at their convenience. This facility for self-printing is applicable for those beneficiaries whose CGHS membership has already been approved by the concerned Additional Director.

MISCELLANEOUS
CGHS card issued by the Directorate General of Central Government Health Scheme(CGHS) is not transferable.
Misuse of CGHS card will entail penal consequences and in the case of Government employees, disciplinary action can also be taken.
The holder of a CGHS card(s) will surrender it to the sponsoring authority when the CGHS card is no longer required by him on account of his superannuation/transfer/or shifting of residence to a non-CGHS covered area.
Each Ministry / Office sponsoring requisitions for CGHS cards shall maintain a register in Form ‘E’ to record the details of CGHS cards issued as per the recommendation of the Ministry / Office. This register shall be submitted to the Directorate General of CGHS if and when required for scrutiny.
