How to Book CGHS Online Appointment

Follow the steps below to book an appointment at a CGHS Wellness Centre or empanelled hospital:

  • Visit the official CGHS website
  • Go to the CGHS beneficiary portal and click on “Book Online Appointment”.
  • Login using Beneficiary ID
  • Enter your Beneficiary ID and validate through the OTP sent to the main card holder’s registered mobile number.
  • Select Wellness Centre and Specialist
  • Choose your preferred CGHS Wellness Centre, specialty (General Duty/Specialist), and the available time slot.
  • Confirm and download appointment slip
  • Once confirmed, save or print the appointment slip. You will also receive an SMS confirmation.
  • Visit the Wellness Centre with required documents
  • Bring your CGHS card/Beneficiary ID, appointment slip, and past medical records.

      Consult your ADMINISTRATION office they will deduct the eligible of your basic pay subscription every month and get the nearest place of your resistance CGHS Wellness Center if your area is not covered they will arrange for it.

    My friend retired in Aug 2004 and he settled in Bareilly, UP. At that time Bareilly was not having CGHS facilities, therefore he didn’t OPT for CGHS CARD . He was getting FMA of ₹1000/- per month, as per 7th pay commission recommendations. He died in Jan2022 and his wife is getting Family Pension. His wife intend to shift to Gurugram, Haryana. Now she is interested in getting CGHS CARD. Please advice how to get it.

    I retried in Sep 2017 from central government department. Due to certain pending matters, I am receiving Provisional Pension regularly based on the Provisional Pension Authorisation issued by the concerned pay and accounts officers. I am not receiving any fixed medical allowance. Kindly inform about the rules and procedure for obtaining CGHS card for self and wife in my case( As I am a Provisional Pensioner). I was informed by a pensioner that CGHS card in my case shall be issued for a period of one year and the same has to be got renewed on yearly bases. Kindly apprise with the factual position and rules.

      What they said is correct. You can pay and get one year card it will be renewed every year by paying every year until regular pension gets.

    Hello Sir,
    My Father was issued this card on the 1st Oct 2020 and unfortunately, he expired in Feb. 2021.
    We had paid the lifetime membership fee of rs 54000, shall we get the balance refund if we are not using this card now.

        Sir,
        I am serving soldier and due for retirement in Oct, me and my family members are eligible to avai ECHS facilities. My father is also a CG Pensioner and not opted for CGHS . As he is dependent on me we have stopped his FMA for opting ECHS.

        Since he is a CG Pensioner & eligible for CGHS, kindly clarify whether he can be included as dependent for ECHS to avail medical facilities or he has to apply for CGHS card ?

          Your father has to apple CGHS CARD. Since his pension was crossed Rs.9000/- he is not dependent to you. But he has to pay as its pension Matrix Level subscription rates multiplies by 12 for every year.

Now I want to change my family to Hyderabad where my daughter was staying and want to avail the facility of CGHS at Hyderabad. Kindly let me know the amount to be paid .

      Yoy have surrender FMA. As per your basic pay you have to pay one year card (250 multiple by 12 )= 3000 or you require life time card 30,000/- at CGHS PAYING AUTHORITIES of your region.

        First you have to surrender FMA. You’ve to pay as per your last retirement subscription amount multiply by 12 months for one year period. If required lifetime card for 120months amount.

      He has to write a letter to the nearest PAO Authority mention to surrendering the FMA and pay the subscription of of Rs.450/- p.m. For one year continuation or 10years contribution for Life time Card. as per his pay MATRIX LEVEL.

      You can file Income tax for the F.Y. 2019-20 with penalty. But there’s no benefit (return amount), you can get reduction from Income TAX if already taxed from that amount as per your eligible.

      You can get the CGHS facility in NON-CGHS residence area for that you have to surrender the Fixed Medical Allowance and get the certificate and pay for. required as per your last basic pay matrix level of subscription amount multiple by 12 months if required for one year. or required Life time Card for 120 months of subscription amount.

      Contact your CGHS zone Authority. Letter will be given for surrendering Fixed Medical Allowance and also pay as per his husband last retired 7th CPC MATRIX LEVEL of subscription (minimum Rs.250/-) for CGHS multiply by 12 months if required one year/120 months for full Life Time Card.

      As per his last subscription in matrix level from the 7th CPC he has to pay from his retirement month i.e. Dec.2016 to Dec,2019 total 37 month plus 120 month total 157 months subscription for LIFE TIME CARD.

      Since you are getting FMA you are not eligible to reimburse the same. You have to surrender the FMA facility & pay for of last subscription 120 months & from the retirement period and get the life time CGHS CARD for full family.

    I am going to retire in May, 2019. I am going to Stay in Madurai, being a non-CGHS station. I want to know after paying an amount of Rs.1,20,000/- for Life time Card, I will get a CGHS Card.
    At the time of emergency, How can I take treatment?
    Every time, coming to Chennai, will be very expensive.
    How can I use this card in a city where there is no CGHS.
    For going to empaneled Labs, how will I get a referrel from Doctor?
    How will I get the medicine?

      There are more empanelled hospitals in Madurai for emergency treatment. If you want CGHS CARD for every year and renew it, after such years you can pay the balance years and make it as life time card .If you don’t require card you can avail the facility of FIXED MEDICAL ALLOWANCE of ₹ 1,000/-p.m.

      In case she pays Rs.1.20lakhs for lifetime CGHS plastic card and likely to avail em-paneled hospital treatment etc, is there any Income Tax rebate can she claim (CGHS subscription she pays Rs1000 p.m while in service has Income tax rebate under Sec80D I suppose with year to year ceiling fixed by the UoI) for the current Assessment year or for future years since Rs.1.2Lakhs is beyond the yearly Sec 80D ceilings.

    Since he got FMA as per his last month subscription multiple by two from the retirement month again multiple by 12 for each year or 120 months subscription for life time card. Before paying get the NOC give letter to ex-employer for getting FMA to stop it.

      First you to give letter to stop FMA from ex-employer & get letter for availing CGHS facility. You have not mentioned your grade pay/7th CPC Matrix level as per the subscription 120 months for life time CGHS CARD. The subscription is in your Service and it will be in double now.

    I have retired from service from 01.01.2011.My grade pay was Rs7600.since my opo book was misplaced by bank,I was given 6 months temporary card by c.g..h.s This continued for 3 instances.subsequently ,I got my p.p.o and I have taken 5 years valid card which would expire on 31.12.2018.Now if I intent to get life time card how much I am to pay .Am I to pay again for 120 months?

    Sir , I was retired on 31 st May 2014 have been taken CGHS card no-4539208,paid for annual subscription valid upto Nov2016, later got it renewed for 2017 & 2018 on annual basis @ Rs 6000/per year, but for the annual subscription of the financial year 2017- 2018 CGHS office demanded me to submit the DD for Rs 12000/-so please advise me how much is to be paid for renewal for 2018-2019.

      Answer to Dr.A.K.SINGH : After give a certificate of not availing fixed medical allowance, if non-resident of CGHS area, they can also get the CGHS card to pay the CGHS subscriptions as per his levels in 7th CPC of 12 months as every year or 120 months for LIFE CARD.

    sir,
    i was retired on 31-12-2015, at the time of retirement my cghs contribution was Rs.500/-, now i want to deposit ten years contribution for getting life time cghs card , how much amount need to be deposited.

      Answer to OM PRAKASH : Now you have to give ₹ 1000/- p.m subscription i.e 1000 × 120 = 1,20,000/- plus current year upto December from the payment of the month. If you are paid earlier I.e. 2/17 it was 50%.only.

    As I informed below, in addition for continuing months you have to pay the CGHS subscriptions of ₹ 1950/- for the 3 months from October to December, 2018.

    As per your matrix level seven your monthly CGHS subscription of ₹ 650 × 120 months for life periods ₹ 78,000/-. Renewal of every year 650 × 12 =7800 per year.

    Sir, I am retiring with effect from September, 2018 on superannuation, Before implementation of 7th pay commission my Grade Pay was Rs.4600/- Now on or before my retirement my Basic Pay will be Rs..55208/- after 3% increment from July, 2018.
    please inform me the total amount to be deposited for availing life time CGHS card.
    I will wait your reply please.
